Million Hearts® and Cholesterol: 2013 ACC/AHA Guideline on the Treatment of Blood Cholesterol to Reduce Atherosclerotic Cardiovascular Risk in Adults: Review of Case Studies Webinar

In late 2013, the American Heart Association and American College of Cardiology released a package of prevention guidelines covering lifestyle, obesity and overweight management, risk assessment and cholesterol management. The cholesterol management guideline presented evidence based recommendations on who should take a cholesterol-lowering statin to prevent more heart attacks, strokes, and premature deaths. The new recommendations are an efficient way of finding people at increased risk who would benefit most from statin therapy. Three high risk groups such as those with heart attack and stroke, those with diabetes ages 40-75 years, and those with a low density lipoprotein (LDL) cholesterol ≥ 190 mg/dl benefit from the appropriate intensity of statin therapy. Recent studies add confirmation that when the decision is made to give a statin for primary prevention based on risk, we are identifying a group who actually benefit. A key feature of the new guidelines is the importance of a clinician-patient discussion that addresses risk factor control, need for adherence to a healthy lifestyle and reviews the potential for benefit as well as the potential for adverse effects and drug interactions. This includes considering factors such as a family history of premature heart and stroke and the possible use of either a coronary calcium score, a hs- CRP (inflammation test), or an ankle brachial index to inform the risk decision if it is uncertain. The clinician-patient discussion concludes with listening to patient preferences before any prescription is written.
